Documents to be submitted upon conclusion of the individual employment contract

Documents to be submitted when concluding an individual employment contract:

• Identity card or another identity document (2 copies);
• Certificates of study that confirm degrees, scientific titles, scientific-didactic titles, honorary titles, and qualification categories (copies of these);
• Photograph 3x4 cm (4 pieces);
• Documents for determining work experience: the work record book, for the period up to December 28, 2018, attestations signed by the heads and chief accountants of the respective institutions, organizations, or enterprises, authenticated with a seal, which must obligatorily reference the document (order, directive, etc.) that confirms work experience;
• Medical certificate, in cases provided by current legislation;
• Self-declaration regarding the fact that during the periods of work at previous workplaces, they did not violate the provisions of Art. 6(2) of Law no. 325 of December 23, 2013 on the evaluation of institutional integrity, except for cases when the person is entering the labor market for the first time;
• Military service documents – for draftees and reservists;
• Job description form (completed by the head of the subdivision, 2 copies).
